About the Role

Provide one-on-one adult skilled nursing care in home and community settings.

Responsibilities

• Deliver direct patient care including assessments, medication administration, wound care, and monitoring chronic conditions.
• Follow individualized care plans and document accurately.
• Collaborate with an interdisciplinary team to ensure optimal patient outcomes.

Requirements

• Licensure as a Licensed Vocational Nurse.
• Strong clinical skills in adult care, wound management, and medication administration.
• Ability to work independently and communicate effectively with team members.
