About the role
Provide skilled adult home health care in patients' homes.
Responsibilities
- Perform comprehensive patient assessments and document in the EHR.
- Administer medications and treatments as prescribed.
- Manage wound care and monitor chronic conditions.
- Collaborate with RNs, therapists, and physicians to follow care plans.
- Educate patients and families to promote safety and independence at home.
Requirements
- Licensed Vocational Nurse (LVN) license required.
- Strong clinical skills in home health care, wound management, and chronic condition monitoring.
- Ability to work collaboratively within a healthcare team.
- Excellent communication and patient education skills.
