About the role

Provide skilled adult home health care in patients' homes.

Responsibilities

• Perform comprehensive patient assessments and document in the EHR.
• Administer medications and treatments as prescribed.
• Manage wound care and monitor chronic conditions.
• Collaborate with RNs, therapists, and physicians to follow care plans.
• Educate patients and families to promote safety and independence at home.

Requirements

• Licensed Vocational Nurse (LVN) license required.
• Strong clinical skills in home health care, wound management, and chronic condition monitoring.
• Ability to work collaboratively within a healthcare team.
• Excellent communication and patient education skills.
