LVN Admissions - Intake Center - Variable Shift

Essential Job Duties

• Reviews documentation related to the patient’s condition and applies accepted criteria to determine if appropriate for admission and appropriate status for the patient upon admission per accepted criteria.
• Communicates appropriately with accepting/admitting physicians and hospital administration to ensure that patient safety and regulatory issues are addressed properly.
• Addresses and resolves issues arising from increased census and decreased bed availability.
• Communicates with the transferring facility to obtain accurate insurance information.
• Communicates with the surgery department, emergency department, nursing units, physician offices and outlying facilities to ensure appropriate placement of patients in a timely manner.
• Evaluates, processes, tracks, documents and coordinates patient transfers for the successful referral and transfer of patients to the facility.
• Ensures admissions, prioritizes/assigns beds based on appropriate level of care and patient aggregation.
• Collaborates with physicians regarding documentation required to accurately describe patient condition to determine to allow for appropriate diagnosis and level of care.
• Maintains appropriate communication channels with all levels of personnel for continuity of care.
• Notifies Directors of particular problems related to patients, families, personnel, or physicians.
• Notifies Administrator-on-call (or designee) of problems requiring administrative intervention.

Required Education And Experience

• Graduate of an accredited nursing program
• 3 - 5 years related experience
