Location

This field-based role enables associates to primarily operate in the field, traveling to client sites or designated locations as their role requires, with occasional office attendance for meetings or training. Candidates must be within a reasonable commuting distance from the posting location(s) unless accommodation is granted as required by law.

Schedule

Monday through Friday, standard business hours.

About the Role

The LTSS Service Coordinator-Clinician works under the direction/supervision of an RN, contributing to the LTSS care coordination process by performing activities within the scope of licensure. This includes assisting the responsible RN with telephonic or face-to-face assessments for the identification, evaluation, coordination, and management of member needs, covering physical health, behavioral health, social services, and long-term services and supports.

Responsibilities

  • Assists responsible RN in identifying members for high-risk complications.
  • Obtains clinical data as directed by the responsible RN.
  • Assists the responsible RN in identifying members that would benefit from an alternative level of care or other waiver programs.
  • Provides all collected information to the responsible RN, who verifies, interprets, and uses it to develop, monitor, evaluate, and revise the member's care plan.
  • Participates in coordinating care for members with chronic illnesses, co-morbidities, and/or disabilities as directed by the responsible RN, ensuring cost-effective and efficient utilization of health benefits.
  • Applies decision-making skills based on member needs, understanding of disease processes, terminology, and clinical guidelines (without requiring nursing judgment).
  • Travels to worksites and other locations as necessary.

Requirements

  • Requires an LPN/LVN, LSW, LCSW, LMSW, or other license (other than RN) in accordance with applicable state law.
  • Nursing Diploma or AS in Nursing or a related field.
  • Minimum of 2 years of experience working with individuals with chronic illnesses, co-morbidities, and/or disabilities in a Service Coordinator or similar role.
  • Current, unrestricted LPN/LVN, LSW, LCSW, LMSW, or other applicable license (as allowed by state law) in the required state(s).
  • May require state-specified certification based on state law and/or contract.

Preferred Qualifications

  • MA/MS in Health/Nursing preferred.

Work Arrangement

Elevance Health operates in a Hybrid Workforce Strategy. Associates are required to work at an Elevance Health location at least once per week, with specific onsite requirements discussed during the hiring process.
