LTSR Nurse / RN / LPN PRN
About the role
This position will be a member of a team that provides support, services, and treatments to individuals with severe and persistent mental illness in a home-like setting. Provides oversight and general nursing care and services to persons with behavioral health challenges by performing the duties listed in this job description. Relies on education, skill, and judgment to provide quality services to the people we serve with limited direction and supervision. Requires an unencumbered Nursing License.
Responsibilities
- Monitor healthcare needs of individuals receiving services and coordinate medical, dental, and vision care services and appointments.
- Provide assistance to individuals receiving services who are unable to self-administer medication and supervise self-administered medication and injection of medications, as prescribed.
- Educate individuals receiving services on the use of their medication including the purpose, side effects, and administration methods.
- Ensure compliance with all internal and external requirements and regulations regarding medication administration documentation and reporting.
- Provide first aid treatment to individuals receiving services, as needed.
- Maintain contact/communication with primary care physicians, dentists, and other healthcare specialists.
- Prepare and complete all necessary documentation, including assessments, nurses notes, medication administration information, and miscellaneous paperwork/reports as requested.
Requirements
Required Education/Experience: Certified Registered Nurse/ LPN, five years’ experience in the human services field working with individuals with behavioral and or mental health issues (preferred).
Required Licenses/Certifications: Valid driver’s license, Valid Nursing License.

$31.50/LPN, $37.00/RN
