Responsibilities

• Conduct on-site licensure inspections, complaint and incident investigations, and annual comprehensive surveys to include the investigation of abuse, neglect, and exploitation at Long-Term Care (LTC) providers to determine compliance with federal and state health and safety regulations.
• Conduct exit conferences at the end of an inspection with facility staff regarding preliminary findings of the inspection.
• Complete written reports for each inspection or survey to detail deficiencies and provide narratives to support regulatory findings.
• Summarize facts obtained during an investigation to determine whether abuse, neglect, or exploitation occurred; ensure documentation is clear and concise; ensure course corrections implemented by providers are appropriate to address risk or noncompliance.
• Make referrals to HHSC's Employee Misconduct Registry of alleged perpetrators when abuse, neglect, or exploitation is determined to have occurred.
• Establish priorities in gathering evidence and determine methods to use in securing evidence, such as interviews or record reviews.
• Analyze evidence to complete thorough investigations and determine if there is a preponderance of evidence; maintain chain of custody and protect evidence during the process.
• Review and approve corrective action plans to address noncompliance; inform providers of issues requiring immediate remedial action to ensure health and safety of consumers; recommend enforcement actions; ensure administrative penalties are assessed and processed according to state and federal requirements.
• Participate in meetings to discuss major issues identified during inspections/investigations and the outcome of inspections/investigations.
• Testify at hearings and court proceedings regarding investigation findings and compliance determinations.
• Train new hires by conducting on-the-job training, including mentoring, shadowing, and return demonstrations.
• Perform other duties as assigned by supervisor or regional leadership, including being available for on-call duties for emergencies such as weather events or high-priority situations at providers.

Requirements

• Valid Class C (or higher) Texas driver's license or equivalent license from another state.
• Experience interacting with or communicating with members of the public, in person or by phone.
• Flexibility in work hours, including evening hours, weekends, and holidays, as required for case-related functions, on-call duties, and/or as required by management.
• Ability to travel up to 80%, including statewide travel, overnight, and weekend travel.
• Reliable transportation.
• Must pass a fingerprint criminal background check and SEMARC (Search Engine for Background Checks) check; additional checks as applicable may include NAR (Nurse Aide Registry) and LNFA (Licensed Nursing Facility Administrator) checks.
• Must disclose any potential conflicts of interest, secondary employment, volunteer activities, and friends/family who own, operate, or work at regulated providers.
• Must bring I-9 documentation on first day of work.

Salary range: $5,035.99 - $6,779.25 per month (Starting Market Rate Salary $60,431.88 annually). Pay frequency: Monthly. Salary Group: TEXAS-B-20.

Schedule

Shift: Day. Full time. Works occasionally outside of traditional working hours (8 am – 5 pm), which includes evening hours, weekends, and holidays, as required for case-related functions, on-call duties, and/or as required by management. Telework: Not Eligible for Telework.
