About the role

Join the Texas Health and Human Services Commission (HHSC) as an Inspector V within the Long-Term Care Regulatory (LTCR) team. This front-line field position involves conducting on-site inspections and investigations at licensed Texas long-term care facilities, including:

• Licensed-only Nursing Facilities
• Assisted Living Facilities
• Day Activity and Health Services (DAHS)
• DAHS-Individualized Skills and Socialization (DAHS-ISS)
• Prescribed Pediatric Extended Care Centers

The role focuses on identifying compliance with federal and state health and safety regulations, with particular emphasis on abuse, neglect, and exploitation of adults and children with mental illness or intellectual, developmental, and physical disabilities. Inspectors analyze evidence, prepare detailed reports, and may testify in hearings or court proceedings. Occasional evening, weekend, and holiday work is required, along with on-call duties for emergencies.

Responsibilities

• Conduct on-site licensure inspections, complaint and incident investigations, and annual comprehensive surveys.
• Investigate allegations of abuse, neglect, and exploitation to determine compliance with regulations.
• Hold exit conferences with facility staff to discuss preliminary findings.
• Write clear, concise reports detailing deficiencies, supporting narratives, and required corrective actions.
• Summarize facts to determine whether abuse, neglect, or exploitation occurred and make referrals to the Employee Misconduct Registry when appropriate.
• Establish priorities for evidence gathering, including interviews and record reviews, ensuring proper chain of custody and protection of evidence.
• Review and approve corrective action plans; ensure timely remedial actions to protect consumer health and safety.
• Assess and process administrative penalties in accordance with state and federal requirements.
• Participate in meetings to discuss major inspection outcomes and provide testimony at hearings or court proceedings.
• Train new hires through on-the-job mentoring, shadowing, and return demonstrations.
• Perform on-call duties for emergencies such as weather events or high-priority provider situations.

Requirements

• Valid Class C (or higher) Texas driver’s license or equivalent from another state.
• Experience interacting with the public, in person or by phone.
• Four-year degree from an accredited college or university preferred.
• Experience conducting investigations preferred.
• Experience working with individuals with mental illness, intellectual or developmental disabilities, or physical disabilities preferred.

Knowledge, Skills, and Abilities

• Ability to interpret local, state, and federal laws and regulations relevant to long-term care.
• Skill in conducting inspections and investigations and determining appropriate courses of action.
• Strong written and verbal communication skills.
• Ability to work independently, manage multiple priorities, and exercise professional judgment.
• Proficiency with Microsoft Office and standard office equipment (computer, tablet, smartphone, fax, copier).
• Ability to conduct interviews in various settings, including private residences, day habilitation facilities, group homes, and state-operated facilities.
• Capacity to work in environments where violent or dangerous individuals may be present.
• Ability to oversee the work of others and integrate complex regulatory concepts.

Pay

Starting market rate salary: $60,431.88 annually. Salary range: $5,035.99 – $6,779.25 per month (salary group TEXAS-B-20).

Schedule

Full-time position with a regular and predictable schedule, though flexibility is required for evening, weekend, and holiday work as needed. On-call duties may be required. Travel up to 80% statewide, including overnight and weekend travel. Reliable transportation is required.
