About the role

Provides skilled nursing care to people recently discharged from the hospital, needing therapy, and those needing long-term care.

Responsibilities

  • Conducts Psychosocial Assessments: Evaluates a new resident's emotional, social, and psychological history.
  • Develops Care Plans: Collaborates with the healthcare team to create and maintain individualized care plans & updates the interdisciplinary team on resident progress and goals.
  • Emotional Support: Provides counseling to help residents cope with grief, loss, depression, anxiety, and the transition to a nursing home.
  • Advocacy: Protects and promotes resident rights, ensuring voices are heard and needs are met.
  • Grievance Resolution: Mediates complaints and reports any suspected abuse or neglect.
  • Discharge Planning: Coordinates safe discharge plans, connecting residents with community resources like home health care or hospice.
  • Resource Connection: Assists with connecting residents and families to legal, financial, and insurance case management.
  • End-of-Life Support: Guides residents and families through advanced directives.

Requirements

Prior Long Term Care Experience

Qualifications

License to work within the State of Connecticut as a LSW

Skills

Able to pass background check, drug screen, professional references check & physical evaluation

High level understanding of leadership, customer service and clinical competency

Leadership experience in a long-term care setting
