About the role
We are seeking a compassionate and skilled LPN to join our weekend shift team at Southern Hills Rehab Center. As an LPN on our team, you will have the opportunity to make a meaningful difference in the lives of our residents. You will work alongside a dedicated team of healthcare professionals to provide high-quality care and support to our residents.
Responsibilities
- Provide direct patient care to our residents, including administering medications, performing wound care, and assisting with activities of daily living
- Maintain accurate and up-to-date records of resident care and treatment
- Monitor and report any changes in resident condition to the charge nurse or physician
- Participate in the development and implementation of individualized care plans for our residents
- Work closely with other members of the healthcare team to ensure seamless transitions and delivery of care
Requirements
- Current LPN license in the state of Oklahoma
- High school diploma or equivalent required
