LPN Weekend Shift

About the role

We are seeking a compassionate and skilled LPN to join our weekend shift team at Southern Hills Rehab Center. As an LPN on our team, you will have the opportunity to make a meaningful difference in the lives of our residents. You will work alongside a dedicated team of healthcare professionals to provide high-quality care and support to our residents.

Responsibilities

• Provide direct patient care to our residents, including administering medications, performing wound care, and assisting with activities of daily living
• Maintain accurate and up-to-date records of resident care and treatment
• Monitor and report any changes in resident condition to the charge nurse or physician
• Participate in the development and implementation of individualized care plans for our residents
• Work closely with other members of the healthcare team to ensure seamless transitions and delivery of care

Requirements

• Current LPN license in the state of Oklahoma
• High school diploma or equivalent required
