LPN Visit Nurse Team - Homecare

Pay: $25 - $28 per visit

About the role

Maxim Healthcare in Hebron is hiring Licensed Practical Nurses (LPN) to work with patients in the homecare setting for a Visit Nurse Position. This role includes quick 35-minute to 1-hour visits in the patient's home, covering tasks such as med pass, insulin assistance, wound care, and more.

We offer three scheduling options:

• Intermittent Nursing Team Tier 1 – Consistent weekly scheduling with an expected number of visits/days per week.
  • Availability: 4 days per week
  • Weekend requirement: Every other weekend or one weekend day per week
  • Minimum visits: 32 per week
• Intermittent Nursing Team Tier 2 – Consistent weekly scheduling with an expected number of visits/days per week.
  • Availability: 3 days per week
  • Weekend requirement: Every other weekend or one weekend day per week
  • Minimum visits: 24 per week
• Casual/PRN option – No set availability expectations.

We are primarily looking for candidates available for evening and weekend shifts. Compensation is discussed on an individual basis.

Responsibilities

• Utilize the nursing process to plan, implement, and evaluate patient/client care and/or services provided to clients receiving contracted services, as required.
• Initiate the plan of care when directed and request revisions from the prescriber as patient status and needs change.
• Incorporate therapeutic, preventive, and rehabilitative nursing actions based on patient diagnosis and established goals.
• Intervene as applicable in response to changes in patient/client condition and report such changes to the appropriate physician or supervisor.
• Administer medications and treatments as prescribed, monitor and document the effectiveness of interventions.
• Review the plan of care and program protocols to identify areas for healthcare teaching/education, care coordination, goal setting, and discharge planning needs.

Requirements

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license for the state in which the nurse practices.
• Current Health Certificate (per facility RN contract or state regulation).
• Current PPD or Chest X-Ray.
• Current BLS card.
