LPN Visit Nurse

Responsibilities

• Provide direct nursing care to acutely or chronically ill patients in their homes, following physician prescriptions.
• Record comprehensive clinical notes and progress reports in electronic medical records.
• Coordinate patient care activities, report changes in patient condition, and participate in care planning and case conferences.
• Ensure compliance with infection control procedures and scope of practice defined by state laws.

Requirements

• Current, unrestricted LPN licensure in the practice state.
• CPR certification and a valid driver’s license with dependable transportation.
• Ability to work independently in diverse home environments and manage multiple tasks.
• Strong communication, organizational, and documentation skills.
