LPN - Various locations in Fairfield County, CT ($5k sign on bonus)

Major Accountabilities / Critical Responsibilities

• Reviews schedules and organizes patient flow; greets patients, documents their concerns, questions, and accompanies them to exam/procedure rooms.
• Initial patient interview; reviews patient medical profiles, and documents all required charting in EMR/manual charting as appropriate.
• Obtains and accurately documents patient vital signs, weight. Reports any abnormalities to the physician.
• Aids with specimen collection and testing as necessary. Documents pertinent information.
• Assists patients as necessary, including walking, transferring, and dressing.
• Provides minor treatment and assists physicians with minor procedures.
• Provides education as needed based on the evaluation of the patient’s or family’s learning needs.
• Schedules patient testing and pre-certifies tests if insurance requirements are applicable.
• Responds to telephone messages from patients, physician offices, etc.
• End of month checks for expired supplies and medications. Stocks rooms/maintains inventory of exam/procedure rooms.
• Orders supplies as needed. Cleans exam/procedure rooms after each patient.
• Prepares exam/procedure rooms for the next patient and ensures physician needs are met.
• Notifies patients of test results (as per physician instructions) and follows up as necessary.

Qualifications/Requirements

• Graduate of an accredited school of nursing.
• Valid license as Licensed Practical Nurse in CT.
• BLS Certification.
• Excellent customer service and communication skills.
• Knowledge of medical terminology.
• Minimum of 2 years previous experience in a similar position preferred.
• Must be able to read, write, and speak the English language.
