Responsibilities
- Aid in the oversight of resident care, assessments, care planning, and nursing services on the assigned unit
- Support and supervise nursing staff, nursing assistants, and unit personnel under the direction of the Unit Manager
- Maintain awareness of resident conditions and promptly notify providers of changes, abnormal results, and care needs
- Ensure accurate and timely completion of physician orders, medications, treatments, laboratory work, and documentation
- Conduct rounds, review reports, address resident needs, and communicate pertinent information during shift changes
- Participate in care plan meetings, family meetings, discharge planning, and interdisciplinary team meetings
- Complete quality assurance audits and support quality improvement initiatives
- Coordinate appointments, transportation, referrals, and continuity of care across disciplines
- Aid in staff education, orientation, policy implementation, and performance management
- Maintain unit supplies, address environmental concerns, track infection control and immunization records, and ensure regulatory compliance
- Respond appropriately to medical, behavioral, and emergency situations
- Perform additional duties as assigned by the Unit Manager or Supervisor
Requirements
- Minimum education requirement: High School Diploma or equivalent; Associate's Degree preferred
- Minimum experience requirement: 1 year of experience as an LPN in a long-term care setting; 3–5 years of LPN experience in long-term care with supervisory experience preferred
- Valid NYS Licensed Practical Nurse (LPN) License required
- Strong clinical assessment, documentation, and care planning skills
- Ability to supervise, coach, and support nursing staff
- Excellent communication, leadership, and customer service skills
- Strong organizational, problem-solving, and critical thinking abilities
- Knowledge of long-term care regulations, infection control practices, and quality assurance processes
- Ability to collaborate effectively with residents, families, providers, and interdisciplinary teams
