About Us

Corning Center for Rehabilitation and Healthcare is a 120-bed skilled nursing facility located in Corning, New York. We serve individuals with skilled nursing needs, postoperative recuperation, or complex medical care demands, as well as chronically-ill individuals who can no longer live independently. Our services include Skilled Nursing Care, Physical Therapy, Occupational Therapy, Rehabilitative Care and Therapy, Social Work Services, and Recreational Activities. Corning Center is a proud member of the Centers Health Care Consortium.

Schedule

• 6AM-2PM
• 2PM-10PM
• On-call responsibilities
• May take a cart when needed

Responsibilities

• Conduct facility rounds.
• Monitor assignments of aides for proper work flow.
• Supervise throughout medication assistance times to ensure proper recording of assistance, refusals, and initiate any necessary follow-up.
• Intervene in resident conflicts, outbursts, and behavior issues.
• Facilitate communication to ensure medical/mental health situations are addressed timely.
• Ensure all aspects of facility incident/accidents are completed and provide a summary to the EHP Coordinator/Administrator.
• Attend to all late admissions/re-admissions (paperwork, medication changes, etc.).
• Communicate all pertinent shift activity via a 24-hour report to be reviewed by the Enriched Housing Program Coordinator/Administrator.
• Facilitate communication for all environmental situations, contact the EHP Coordinator/Administrator when necessary, and record all details for review.

Requirements

• Must be able to work as a team member.
• Valid state LPN license.
• Must be in good standing with State Registry.
