LPN, Scranton Transitional Rehab Unit (PT, Evenings)

About the Role

The Licensed Practical Nurse (LPN) administers direct nursing care to patients and performs a variety of indirect patient care duties under the supervision of the RN Supervisor of the Transitional Care Unit. The LPN is responsible for adhering to all professional working policies and demonstrating knowledge and skills necessary to provide care appropriate to patients from adolescence through geriatric age groups.

The LPN must understand principles of growth and development over the life span, assess patient data, interpret age-specific needs, and provide care as outlined in the Nursing Department's policies and procedures.

Responsibilities

• Administer direct nursing care to patients in the Transitional Care Unit.
• Perform indirect patient care duties as required.
• Assess and interpret patient data reflective of their status and age-specific needs.
• Provide individualized care appropriate to the age and cultural needs of patients.
• Implement policies and procedures of the Nursing Department and Allied Services.
• Maintain standards of nursing care in compliance with professional guidelines.

Requirements

• Graduate of an accredited Licensed Practical Nurse Program.
• Completion of a pharmacology course (minimum 45 hours) included in the basic education curriculum or attended post-graduation.
• Current license as a Practical Nurse issued by the Pennsylvania State Board of Nurse Examiners.

Qualifications

• Minimum of 2 years of experience in a long-term care setting preferred.
• Ability to demonstrate knowledge and skills necessary for providing age-appropriate and culturally sensitive care.

Physical Demands

• Medium Work: Exerting 25 to 50 lbs. of force occasionally (10-33% of shift), 10-25 lbs. of force frequently (33-66% of the time), and up to 10 lbs. of force constantly.
• Frequent bending, reaching, stooping, and lifting (occasionally in strained positions) throughout the shift.
• Ability to transport self throughout the facility.

Working Conditions

• Works in a well-lit nursing unit, including nurses' stations, patient rooms, and treatment areas.
• Occasional exposure to blood and body substances; possible exposure to needle sticks.
