LPN / RN Home Health Nurse

About the Role

Angels of Care Home Health is seeking Registered Nurses (RN) and Licensed Practical Nurses (LPN) to provide direct, one-on-one medical care to medically fragile infants, children, and adolescents primarily in their homes in Surprise, AZ and surrounding areas. We offer full-time and part-time shifts, including days, nights, and weekends.

Responsibilities

• Focus on the medical needs and treatment of patients according to the active plan of care, as directed by the physician and other appropriate personnel.
• Conduct ongoing patient care and assessments.
• Administer prescribed medication, treatments, and therapies.
• Coordinate patient care.
• Educate family members on patient clinical care to enhance positive outcomes.
• Implement preventative initiatives to protect and improve the quality of care for patients.

Requirements

• Active RN or LPN/LVN license (New Grads Welcome; training provided).
• Ability to provide care in a client home setting.
• Ability to make a positive and lasting impression.
