LPN/RN Home Care (Peds to Young Adults)

About the role

Private Duty Nursing at Phoenix Home Care & Hospice offers a focused, one-on-one environment where your skills and presence provide a steady hand, allowing families to stay together. You'll deliver expert, specialized care to children and young adults with complex medical needs, building deep connections with patients and their families while helping them thrive at home.

Shift differentials:

• Weekday Overnights: +$2/hour
• Weekend Days: +$3/hour
• Weekend Overnights: +$4/hour

Responsibilities

• Provide skilled nursing for pediatric patients with complex medical needs (pediatric experience is a plus, but specialized training is provided)
• Build genuine bonds with patients and become a trusted partner to their families
• Deliver care in the patient’s home, creating a sanctuary of safety and comfort

Requirements

• Current and active LPN or RN license
• Passion for patient-centered care and a desire to make a visible impact (pediatric experience is a plus but not required)
