LPN Rehab Nurse | Full-Time

Role Summary Provides basic nursing care by assessing residents upon admission and continuously monitoring their condition. Communicates significant changes to the interdisciplinary team and documents all assessments accurately. Participates in care planning, updating plans as needed, and ensuring effective communication with residents and families. Implements nursing interventions, including medication administration and treatments, adhering to physician and nursing orders. Requires IV certification for IV fluid administration and ensures timely, safe delivery of care. Utilizes critical thinking to recognize changes in residents' conditions, promptly notifying physicians using SBARR communication. Maintains professionalism during crises, monitors care effectiveness, and addresses problem areas through the QAPI process. Responds to call lights and assists with activities of daily living (ADLs). Completes ongoing nursing education to stay current with best practices and may be assigned additional duties as appropriate. Qualifications & Skills Licensed Practical Nurse (LPN) license in the relevant stateExperience in long-term care, rehab, or assisted living preferredIV and CPR/AED certification required or willing to obtain within six monthsAbility to assess geriatric nursing care needsExcellent communication, time management, and organizational skillsStrong critical thinking abilities
