Job Summary

We are seeking a compassionate and dedicated LPN Private Duty Nurse to provide professional, high-quality care to our patients in the comfort of their homes.

Key Responsibilities

• Provide direct nursing care to patients in their homes according to their care plan.
• Assist with activities of daily living (ADLs).
• Develop and implement individualized care plans.
• Perform wound care, catheter care, and other necessary medical tasks.
• Maintain accurate and timely records of patient care.
• Ensure patient comfort and safety in the home environment.
• Provide emotional support and companionship to the patient and family.
• Collaborate with other healthcare professionals.

Qualifications

• Licensed Practical Nurse (LPN) with a valid state license.
• Proven experience in home care or long-term care settings preferred.
• Experience in wound care, medication administration, and performing nursing assessments.
• Ability to work independently with minimal supervision.
• Strong communication and interpersonal skills.
• Compassionate, patient, and empathetic nature.
• Ability to handle emergency situations calmly and effectively.
• Knowledge of home healthcare regulations and best practices.

Physical Requirements

• Ability to lift, move, and transfer patients when necessary.
• Ability to stand, walk, stoop and bend for extended periods.
• Ability to perform basic physical tasks.
• Good visual acuity and hearing.
