Home and Community Based Services Licensed Practical Nurse Role: Provide direct nursing care to acutely and chronically ill patients in their homes, following physician prescriptions. Responsibilities include documenting patient visits, coordinating care activities, and ensuring high-quality care within legal and regulatory standards. Work under supervision of a Registered Nurse or designated supervisor. Key Duties Deliver clinical services within the scope of practice, adhering to infection control protocols.Thoroughly document patient visits in electronic medical records and visit verification systems.Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members. Qualifications & Skills Unrestricted LPN licensure and current CPR certification.Valid driver’s license, reliable transportation or access to public transit.Ability to work effectively in diverse home environments and independently.Preferred experience in home care, strong communication, organizational, and multitasking skills. Compensation & Benefits: Hourly pay ranges from $22.90 to $34.35 based on experience and qualifications.
