Role Overview

The Home and Community Based Services Licensed Practical Nurse (LPN) provides direct nursing care to acutely or chronically ill patients in their homes, under supervision.

Responsibilities

• Deliver high-quality clinical services within the scope of practice, adhering to state laws and the plan of care.
• Document patient visits thoroughly in electronic medical records, including electronic visit verification when applicable.
• Participate actively in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, physicians, and team members.

Qualifications & Skills

• Current, unrestricted LPN licensure in the state of practice.
• CPR certification and access to reliable transportation or public transit.
• Ability to care for patients in diverse home environments, demonstrating flexibility and independence.
• Preferred: home care experience, strong communication and organizational skills, and capacity to manage multiple tasks.
