About the Role
The Home and Community Based Services Licensed Practical Nurse provides direct nursing care in patients' homes, following physician prescriptions.
Responsibilities
- Deliver high-quality clinical services within legal scope, adhering to the care plan and infection control protocols.
- Document patient visits accurately in electronic medical records and visit verification systems.
- Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.
Requirements
- Current LPN licensure in the state of practice.
- CPR certification and a valid driver’s license with reliable transportation.
- Ability to work in diverse home environments without discrimination.
