About the role

Nemours is seeking a full-time Licensed Practical Nurse to join our Primary Care team in Palm Bay, Florida. Nemours primary care practices provide a kid-friendly, family-centered pediatric primary care experience for families in Central Florida. With 20 primary care and three urgent care practices conveniently located throughout the region, we provide quality, professional pediatric services with special attention on preventive care and whole-child wellness.

Responsibilities

• Act as a liaison between the patient and physician/provider.
• Assist physicians, other nurses, and additional medical care team members in the delivery of care to pediatric patients.
• Perform complete and accurate patient assessment or reassessment.
• Screen patients for appropriate information and instruct them in the collection of specimens.
• Provide education to the patient and/or parent/guardian in a manner that the recipient understands to contribute to positive patient care outcomes and patient satisfaction.
• Coordinate clinic flow:
  • Greet patients and families and prepare them for provider examination.
  • Confirm patient ID using two identifiers (name and DOB) and room patients.
  • Obtain vital signs.
  • Obtain and prepare laboratory specimens.
  • Perform in-office laboratory tests and enter results in patient chart.
  • Perform laboratory controls and ensure they are within range; troubleshoot any out-of-range control.
  • Administer immunizations according to guidelines.
  • Administer prescribed medications including oral, injectable, and inhaled according to guidelines.
  • Review and prepare charts for clinic visits.
• Perform spirometry and troubleshoot equipment.
• Actively participate in team huddles and exhibit 200% accountability.
• Maintain and review medical records and other patient information, ensuring appropriate documentation is available for provider review.
• Check laboratory results, post tests, and exam results are in the chart as required.
• Follow up on cultures, labs, outside testing ordered, and discharge summaries (hospital/ER/UC).
• Enter and complete telephone encounters per protocol.
• Ensure in-basket pools are completed timely.
• Monitor voicemail and preview patient charts to assist with identifying gaps in care.
• Prepare equipment and rooms and assist physician/provider during treatments, examinations, procedures, and testing of patients.
• Maintain clinical supplies and organize exam rooms:
  • Keep exam rooms clean and stocked with all needed supplies.
  • Ensure medications and immunizations are safely stored in an appropriate area as designated by law and departmental policy.
  • Evaluate medication and immunization expiration dates on a monthly schedule.
• Ensure provider's orders are carried out.
• Arrange patient diagnostic testing at outside facilities, including completion of prior authorization.
• Schedule referrals for appointments, home care, radiology, and other requests.
• Call pharmacies for prescription orders/refills.
• Work Healthy Planet reports and Care Gaps.
• Respond to phone calls and give advice as dictated, according to established guidelines.
• Work with schedulers to schedule patients according to orders.
• Answer or return calls in a courteous and timely manner and document calls according to established guidelines.

Requirements

• Specialized training (1 year beyond high school) required.
• Minimum of one (1) year of experience as a Licensed Practical Nurse.
• Current Florida LPN license.
• Basic Life Support (BLS) certification required.
