About the role
Nemours is seeking a Licensed Practical Nurse (Primary Care, Palm Bay) to join our Primary Care team in Palm Bay, Florida.
Responsibilities
- Acts as a liaison between the patient and physician/provider.
- Aids physicians, other nurses, and additional medical care team members in delivering care to pediatric patients.
- Performs complete and accurate patient assessments or reassessments.
- Screens patients for appropriate information and instructs them in specimen collection.
- Provides education to patients and/or parents/guardians in a manner they understand to contribute to positive patient care outcomes and satisfaction.
- Coordinates clinic flow: greets patients and families, prepares them for provider examination, confirms patient ID using two identifiers (name and DOB), and rooms patients.
- Obtains vital signs, obtains and prepares laboratory specimens, performs in-office laboratory tests, enters results in patient chart, performs laboratory controls, and ensures results are within range.
- Administers immunizations according to guidelines, administers prescribed medications, and trouble shoots any out-of-range control.
- Reviews and prepares charts for clinic visits, performs spirometry, and trouble shoots equipment.
- Participates actively in team huddles, exhibits 200% accountability, maintains and reviews medical records, and ensures appropriate documentation is available for provider review.
- Checks laboratory results, post-tests, and exam results in the chart as required, follows up on cultures, labs, outside testing ordered, and discharge summaries (hospital/ER/UC).
- Enters and completes telephone encounters per protocol, ensures in-basket pools are completed timely, monitors voicemail, previews patient charts, and assists with identifying gaps in care.
- Prepares equipment and rooms, assists physician/provider during treatments, examinations, procedures, and testing of patients, maintains clinical supplies, keeps exam rooms clean and stocked, ensures medications and immunizations are safely stored, and evaluates medication and immunization expiration dates on a monthly schedule.
- Arranges patient diagnostic testing at outside facilities, including completion of prior authorization, schedules referrals for appointments, home care, radiology, and other requests, calls pharmacies for prescription orders/refills, works Healthy Planet reports and Care Gaps, responds to phone calls and gives advice as dictated, according to established guidelines, and schedules patients according to orders.
Requirements
- Specialized (1 year of training beyond high school) required.
- Minimum of one (1) year experience required.
- Valid State of Florida LPN License required.
- AHA BLS Certification required upon hire.
