About the role

A Licensed Practical Nurse (LPN) provides essential patient care in an outpatient setting, working closely with healthcare providers to promote health and wellness.

Responsibilities

  • Document all patient care activities accurately.
  • Gather patient data, including vital signs, history, and chief complaints, and assess medication refill needs.
  • Collect data for compliance with JCAHO requirements, such as pain and domestic violence screening.
  • Prepare patients for procedures, tests, and treatments, assisting healthcare providers as needed.
  • Evaluate patient needs and risks based on age, health, and medications.
  • Assist with supplies, equipment, and facility assessments.

Qualifications

  • Graduation from an accredited LPN program.
  • Current New York State license and registration to practice as an LPN.
  • Infection control and BLS certifications.
  • Strong communication skills, efficient task completion, and ability to react appropriately in emergencies.
  • Commitment to ongoing professional development and staying current with nursing practices.
