About the role

We are currently seeking a Licensed Practical Nurse (LPN) / Charge Nurse to serve in our Personal Care / Assisted Living neighborhood. In this role, you’ll be part of a dynamic and compassionate team that supports elderly residents as they transition from independent living to assisted care. Your presence will help uphold the caring culture of Fairmount, where every resident is treated with dignity and respect.

PRN hours only, with a requirement of two 8-hour shifts per month.

Pay

$33.20 - $35.80 per hour, with shift differentials: $1.25 per hour for weekend shifts, $1.00 per hour for evening shifts, and $0.50 per hour for night shifts.

Responsibilities

• Accounts for narcotics for the assigned neighborhoods.
• Receives report from the previous shift’s charge nurse for assigned neighborhoods and gives the same to the subsequent shift’s Charge Nurse. Notifies Resident Assistants of any unusual or additional care needed for residents assigned to them.
• Administers medications and completes treatments per physician’s orders following the Pharmacy Policy and Procedure Manual.
• Makes rounds, checking all residents. Completes nursing documentation on residents' records (e.g., progress notes, flow sheets, monthly summaries).
• Assigns additional duties to Resident Assistants (e.g., weekly supply orders and PRN, cleaning chores).
• Assists with resident assessments, admissions, discharges, transfers, and deaths.
• Receives written, faxed, and verbal orders per facility policy.
• Maintains safety and infection control standards following the regulations of State and Federal regulatory agencies.
• Initiates Incident and Accident reports per policy.
• Keeps Director of Personal Care informed of the status of residents, including all incidents and accidents.
• Supervises Resident Assistants and requires accountability to comply with the facility's policies and procedures.
• Responsible for attending mandatory meetings.
• Adheres to Corporate Compliance Plan.
• Complies with HIPAA Privacy standards.
• Completes additional tasks as required to ensure the highest quality care is maintained.
