About the role

We are seeking a Licensed Practical Nurse (LPN) / Charge Nurse for our Personal Care / Assisted Living neighborhood. This PRN position requires a minimum of two 8-hour shifts per month. Responsibilities

• Account for narcotics in assigned neighborhoods.
• Receive and give shift reports to ensure continuity of care.
• Notify Resident Assistants of any unusual or additional care needs for residents.
• Administer medications and complete treatments per physician’s orders, following the Pharmacy Policy and Procedure Manual.
• Conduct resident rounds and complete nursing documentation (e.g., progress notes, flow sheets, monthly summaries).
• Assign duties to Resident Assistants, including supply orders and cleaning tasks.
• Assist with resident assessments, admissions, discharges, transfers, and deaths.
• Receive and process written, faxed, and verbal orders per facility policy.
• Maintain safety and infection control standards as regulated by state and federal agencies.
• Initiate Incident and Accident reports as required by policy.
• Keep the Director of Personal Care informed of resident status, including all incidents and accidents.
• Supervise Resident Assistants and ensure compliance with facility policies and procedures.
• Attend mandatory meetings and adhere to the Corporate Compliance Plan.
• Comply with HIPAA Privacy standards.
• Complete additional tasks to maintain high-quality care.

Requirements

• Current Licensed Practical Nurse (LPN) license with the Commonwealth of Pennsylvania.

Physical and Composure Requirements

The role involves frequent physical activities, including:

• Balancing, standing, walking, and sitting.
• Occasional climbing, pulling, pushing, and kneeling.
• Frequent crouching, grasping, stooping, and lifting (up to 50 pounds).
• Rare fine finger movement and repetitive movement.

Sensory and communicative activities include daily use of hearing, seeing, smelling, feeling, and speaking. The role requires composure to remain calm and professional under stress, including handling frustrated or angry staff/residents and emergency situations.

Pay

• Hourly wage: $33.20 - $35.80.
• Additional differentials: $1.25/hour for weekend shifts, $1.00/hour for evening shifts, and $0.50/hour for night shifts.

Schedule

PRN hours with a minimum requirement of two 8-hour shifts per month.
