About the role
Provide direct nursing care to acutely and chronically ill patients in their homes, under supervision.
Responsibilities
- Deliver high-quality clinical services within legal scope, following care plans and infection control protocols.
- Document patient visits thoroughly in electronic medical records and visit verification systems.
- Participate in care coordination by reporting patient needs, attending case conferences, and communicating effectively with patients, families, and healthcare team members.
Requirements
- Current and unrestricted LPN licensure in the state of practice.
- CPR certification, valid driver’s license, and access to reliable transportation.
- Ability to work independently in diverse home environments, managing multiple tasks with strong communication and organizational skills.
