About the Role

Concierge Home Care is hiring a Home Health Licensed Practical Nurse (LPN) to provide skilled nursing care in Lutz/West Hillsborough County, FL. The role offers flexible scheduling, 1:1 patient care in the home, no nights or on‑call, and territories designed to minimize drive time. Nurses enjoy autonomy to create and manage their own daily schedule, with options for full‑time, PRN, or modified schedules. The position uses AI‑assisted charting and a user‑friendly EMR (WellSky/Kinnser) to streamline documentation, and includes structured onboarding, mentorship, ongoing clinical training, free CEUs, and career advancement opportunities.

Responsibilities

• Provide skilled nursing care under the supervision of a Registered Nurse
• Perform patient assessments and assist with care planning
• Administer medications and treatments as ordered
• Educate patients and caregivers on disease management and care plans
• Monitor patient condition and report changes to the supervising RN
• Collaborate with physicians and interdisciplinary teams
• Complete documentation using streamlined, AI‑assisted tools
• Utilize AI‑assisted charting and user‑friendly EMR (WellSky/Kinnser) to finish notes faster
• Participate in structured onboarding, mentorship, ongoing clinical training, and free CEUs for professional growth

Qualifications

• Active, unencumbered Florida Licensed Practical Nurse (LPN) license
• Valid driver’s license, reliable transportation, and auto insurance
• CPR certification
• Home health experience preferred, but not required

Pay

• Pay‑per‑visit model: $30–$35 per visit (base rate)
• Average full‑time earnings: $55,000–$65,000+ annually
