Coverage Area: Heathrow/ Northwest Orange and Seminole Counties (Territories designed to minimize drive time with clustered scheduling).

Responsibilities

• Provide skilled nursing care under the supervision of a Registered Nurse
• Perform patient assessments and assist with care planning
• Administer medications and treatments as ordered
• Educate patients and caregivers on disease management and care plans
• Monitor patient condition and report changes to the supervising RN
• Collaborate with physicians and interdisciplinary teams
• Complete documentation using streamlined, AI-assisted tools

Qualifications

• Active, unencumbered Florida Licensed Practical Nurse (LPN) license
• Valid driver’s license, reliable transportation, and auto insurance
• CPR certification
• Home health experience preferred, but not required
• Successful completion of an AHCA Level 2 background screening may be required

Pay

Pay‑per‑visit model: $30–$34 per visit (base rate) with quarterly bonus opportunities.

Schedule
