About the role
Licensed Practice Nurse (LPN) providing in-home patient care.
Responsibilities
- Assess, plan, and implement patient care.
- Educate patients and families on disease management, procedures, and medications.
- Manage needs such as wound care, tracheostomy, G-tubes, catheters, ventilators, and medication administration.
- Create customized care plans based on individual needs and family requests.
Qualifications & Skills
- LPN licensure.
- Ability to work independently.
- Strong communication and clinical skills.
- New graduates are welcome and offered comprehensive training, including lab, shadowing, and hands-on experiences.
