LPN Patient Care Coordinator - Home Care/Hospice
Responsibilities
Screens all inquiries from referral sources (hospitals, physicians, clinics, patients/family members, etc.) and provides accurate and reliable information regarding the criteria for admission to home health care/hospice services.
Responds knowledgeably and professionally to inquiries regarding Rochester Regional Health Home Care and Hospice services and programs.
Obtains necessary patient specific information at the time of referral to maximize the potential that patient will meet admission criteria.
Accurately inputs admission data and completes referral and coordinates service requests with a focus on the patients’ care needs and to assure a timely admission to service.
Understands and utilizes Medicare home care guidelines in establishing home care referrals.
Captures, documents, and provides reports of required data.
Builds and maintains productive working relationships with all internal and external customers.
Provides assistance when needed with referral process for Home Care Coordinators.
Assists with special projects, activities.
Consistently demonstrates high standards of integrity by supporting the Rochester Regional Health's mission and values and adhering to the Corporate Code of Conduct.
Maintains high regard for member privacy in accordance with the corporate privacy policies and procedures.
Qualifications
Preferred Qualifications:
- 2 years home health care LPN experience preferred.
- Maintains knowledge of insurance and reimbursement guidelines and changes.
- Strong communication skills.
- Strong decision-making and problem-solving skills.
- Excellent time management and organizational skills.
- Ability to handle changing work environment and to prioritize activities.
- Pioneered ability to work in a team environment.
- Flexibility to work a varied work schedule when needed to meet department needs.
- Basic computer skills are necessary, including but not limited to the ability to navigate on a personal computer within a Windows based operating environment; the ability to access and create basic e-mail messages; and the ability to open, edit, and save basic electronic documents if necessary.
Required Qualifications:
- Graduate of an accredited Licensed Practical Nursing program with a minimum of 2 years LPN experience required.
- A minimum of 2 years customer service experience required.
