We are seeking a Hospice LPN Patient Care Coordinator to manage the day-to-day operations of our hospice programs and coordinate care services across clinical teams. This pivotal role supports timely and compassionate patient care by scheduling visits, facilitating interdisciplinary team meetings, verifying eligibility, and assisting with skilled nursing care as needed—all while ensuring compliance with regulatory standards.
