About the role
Licensed Practical Nurse (LPN) providing skilled nursing care to pediatric and/or adult patients in their homes.
Responsibilities
- Assess, plan, implement, and evaluate patient care using the nursing process.
- Provide skilled nursing services according to prescribed orders.
- Monitor signs and symptoms indicating physiological and psychosocial changes.
- Collect, analyze, and interpret data, documenting nursing diagnoses.
Qualifications & Skills
- Current LPN/LVN license in practicing state.
- Current TB or Chest X-Ray and BLS certification.
- Preferably one year of professional nursing experience.
Key Duties
- Deliver comprehensive nursing care tailored to patient needs, ensuring safety, accuracy, and adherence to medical orders.
- Assess patient conditions, interpret data, and communicate effectively with healthcare team.
