Responsibilities
- Implement plans of care as assigned by the Provider and Primary RN, collaborating with the Interdisciplinary Team.
- Carry out nursing tasks as directed by the RN Manager, including receiving calls from participants and their caregivers, conducting appropriate follow-up, and timely documentation in the medical record.
- Administer prescribed medications to participants attending the PACE center and make frequent home visits in the community to evaluate the home environment.
- Fill weekly medication packs, record vitals, administer prescribed treatments, and deliver relevant health teaching for participants and their caregivers.
- Make recommendations to the Interdisciplinary Team regarding unmet participant needs.
- Work closely with the RN Manager and other nursing staff to coordinate all care for SE participants.
- Occasionally assist other SE PACE centers in person or remotely, as appropriate, when there is an opening or a gap in coverage.
Qualifications
- Graduate of an Accredited Practical Nursing Program.
- Valid license to perform Practical Nursing in the Commonwealth of Massachusetts.
- Willingness to assist other SE PACE centers as needed.
- Certification in CPR/Healthcare Provider issued by The American Heart Association or another accredited agency.
- Driving a motor vehicle is an essential job function, requiring a valid drivers' license, satisfactory drivers' record report, minimum state-required insurance, and reliable transportation.
- Maintain compliance with Fallon Health's Drivers' Safety Policy.
- At least three years of nursing experience in a primary care practice or long-term care facility, or similar clinical setting.
- At least one year of experience working in a nursing capacity with frail elders.
