About the role
The Barton House is a 20-bed memory care community dedicated to caring for individuals living with Alzheimer’s disease and other forms of dementia. Our setting offers private studio rooms, home-cooked meals, meaningful activities, and compassionate care from a team that treats residents like family.
Responsibilities
- Administer medications and treatments in accordance with physician orders and state regulations
- Monitor residents for changes in physical or cognitive condition and report concerns promptly
- Maintain accurate and timely clinical documentation
- Assist with resident assessments and care plan updates as needed
- Communicate effectively with families, physicians, pharmacies, and outside healthcare providers
- Support and guide caregivers and medication aides during shifts
- Respond to resident health concerns and emergencies appropriately
- Ensure compliance with assisted living and memory care regulations
- Participate in care conferences and family communication when needed
- Affiliate with staff on proper care techniques and dementia care best practices
- Promote a positive, respectful, and team-oriented work environment
Qualifications
- Licensed Practical Nurse (LPN) or Registered Nurse (RN) in good standing
- Experience in geriatrics, assisted living, memory care, or long-term care preferred
- Knowledge of medication administration and documentation practices
- Strong clinical observation and communication skills
- Compassion, patience, and a genuine passion for caring for individuals living with dementia
- Ability to work collaboratively with caregivers, leadership, and families
