About the role
We will train new grads! Full-time positions available.
Responsibilities
- Provide personalized, compassionate care in alignment with care plans and physician orders
- Work closely with our Nurse Case Manager and interdisciplinary care team
- Communicate clearly with patients, families, physicians, and staff
- Monitor and report on physical, emotional, and social health factors
Requirements
- Active LPN/LVN license in Arizona
- Excellent communication and organizational skills
- Insured, reliable vehicle and valid driver's license
- Current BLS certification
- Home Health experience and bilingual (English/Spanish) preferred
- Fingerprint Clearance Card (or willingness to obtain)
