Suncoast Hospice, a member of Empath Health is seeking a caring and motivated Hospice Care Center LPN to work with us at our Care Center while providing exceptional care. Schedule: Day shift 8a-8p every other weekend Since 1977, Suncoast Hospice has been honored to serve our neighbors across Pinellas and Hillsborough counties with compassionate end-of-life care. As part of Empath Health, we help those living with advanced illness find comfort, dignity, and meaning, while supporting their families and caregivers with guidance and care every step of the way. What You'll Do Collaborates with the designated off site RN related to issues beyond the scope of an LPN. Paricipates in patienb'caregiver education and discharge planning. Wntains clinical documentation accurately, using professional terminology protocol to ensure continuity of patient care. Collaborates cooperatively with RN and Certified Nursing Assistant throughout shift. Attends patient deaths, provides bereavement support to the patient caregiver/ family, as indicated and performs post mortem care. Why Join Empath Health? What You'll Need Graduation from an approved school of practicallvocational nursing. Current license in State of Florida.
