LPN – Nursing Home Day Shift

About the role

The Licensed Practical Nurse (LPN) at Sedgwick County Memorial Nursing Home provides compassionate, resident-focused nursing care in our 32-bed intermediate care facility.

Responsibilities

• Observes, monitors, and accurately documents resident symptoms, behaviors, reactions, and changes in condition, promptly reporting concerns to the Charge Nurse, Director of Nursing, or provider.
• Administers medications and treatments and assists providers with procedures in accordance with clinical orders, scope of practice, and facility policy.
• Completes comprehensive nursing admissions, ensuring all required forms, assessments, and documentation are accurate, timely, and consistent with regulatory and facility standards.
• Utilizes appropriate requisitions, forms, and clinical records to support resident care, maintaining familiarity with documentation tools and workflows used across nursing units as applicable.
• Actively participates in resident care planning by contributing observations, suggestions, and interdisciplinary insights to support individualized, person-centered plans of care.
• Adheres to established lines of communication and escalation pathways to support effective problem-solving, teamwork, and resident safety.
• Maintains open, respectful, and professional communication with residents, families, physicians, leadership, CNAs, and all members of the healthcare team to promote a positive care environment.

Qualifications

• Graduate of an accredited School of Nursing and current Licensed Practical Nurse (LPN) license in the State of Colorado (or compact state eligibility).
• Currrent BLS certification required.
• Prior experience in long-term care, skilled nursing, or geriatric care preferred.
• Strong commitment to resident dignity, safety, and person-centered care principles.
• Proficient in accurate medication administration, treatment performance, documentation, and follow-through.
• Skilled in monitoring residents for changes in condition, identifying early warning signs, and notifying appropriate personnel.
• Strong computer and charting skills, including EMR use for documentation and communication.
• Effective communication and interpersonal skills for engaging with residents, families, CNAs, providers, and interdisciplinary staff.
• Skilled in time management, prioritization, delegation, and working efficiently during varying workloads.
• Ability to maintain professionalism, compassion, and patience when supporting residents with cognitive, behavioral, or functional challenges.
• Working knowledge of long-term care nursing practices, including medication administration, treatments, restorative care, and resident observation.
• Understanding of resident rights, abuse prevention, infection control, pressure injury prevention, fall prevention, and other major clinical risk areas in long-term care.
• Knowledge of regulatory requirements including CMS long-term care guidelines, HIPAA, and facility policies and procedures.
• Familiarity with care planning, documentation standards, and electronic medical record (EMR) workflows.
• Awareness of emergency procedures, basic life support principles, and clinical escalation pathways.
