LPN Nurse Care Manager

Responsibilities

• Provide health education to clients in individual and group settings
• Maintain prescriber scheduling with specific attention to provider productivity
• Review lab work and coordinate with prescribers, as necessary
• Document all nursing activities provided
• Support clients through nursing case management
• Support prescribers with clerical support
• Administer medications, including injectable medications, while maintaining precise records
• Provide emergency medical care to clients until emergency services arrive, when necessary
• Coordinate client care with internal and external providers
• Perform other duties as assigned

Qualifications

• A High School Diploma or equivalent
• LPN certification
• Valid Driver's License in good standing
