About the role

The Licensed Practical Nurse (LPN) role is an integral part of a multidisciplinary clinical team, responsible for delivering safe, evidence-based patient care in an outpatient medical office setting. The LPN supports clinicians, anticipates the needs of both patients and providers, and helps create a respectful, organized, and compassionate care environment for a diverse patient population across community health centers.

Responsibilities

  • Monitor, document, and clearly communicate patients’ vital signs and measurements to the clinical team.
  • Observe patients closely and promptly note any physical or emotional changes, escalating concerns as needed.
  • Prepare examination rooms before each visit, ensuring they are clean, organized, and appropriately stocked.
  • Select, prepare, and manage medical supplies and equipment required for examinations and treatments.
  • Maintain a patient environment that is comfortable, organized, safe, and hygienic at all times.
  • Accurately maintain and update patient records, including documentation in the electronic medical record (EMR).
  • Prepare patients for follow-up care after treatments or procedures, providing clear instructions and support.
  • Serve as a knowledgeable resource on nursing-related issues within the section or department.
  • Potentially guide, mentor, and educate other LPNs to support consistent, high-quality nursing practice.
  • Organize and manage scheduling for patients, including coordinating necessary medical tests and follow-up appointments.
  • Collaborate closely with clinicians and other healthcare team members to ensure coordinated, patient-centered care.
  • Demonstrate reliable attendance and punctuality to support continuity of care and clinic operations.

Requirements

  • Active Licensed Practical Nurse (LPN) license in the state of New York.
  • 2+ years of experience as an LPN in a clinical or healthcare setting.
  • 1–2 years of medical office experience, preferably in outpatient or ambulatory care.
  • Hands-on patient care experience in a nursing clinical environment.
  • Proficiency with electronic medical records (EMR) systems.
  • Strong attendance, reliability, and dependability in a fast-paced clinical setting.
  • Ability to provide safe, evidence-based nursing care under the direction of clinicians.
  • Effective communication skills to document and relay patient information accurately.
  • Able to organize tasks, manage time, and prioritize patient needs efficiently.

Qualifications

  • Basic Life Support (BLS) certification is a plus.
  • Experience working in a community health or primary care environment is beneficial.
  • Comfort working with diverse patient populations, including underserved and vulnerable communities.
  • Able to educate and support patients regarding follow-up care and treatment plans.
  • Willingness to guide and support other LPNs and team members when needed.
  • Strong interpersonal skills and a patient, compassionate approach to care.
  • Able to adapt to changing clinical needs and schedules in a multi-site health center network.

Skills

  • Active Licensed Practical Nurse (LPN) license in the state of New York.

Pay and Benefits

The pay range for this position is $28.00 - $35.00/hr. Eligibility requirements apply to some benefits and may depend on your job classification and length of employment. Benefits are subject to change and may be subject to specific elections, plan, or program terms.
