About the role
Registered Nurse (RN) or Licensed Practical Nurse (LPN) on the Transition team, providing home care during a provisional period of 1–90 days to facilitate patient discharge and transition to full-time caregivers.
Responsibilities
- Utilize the nursing process to assess, plan, implement, and evaluate patient care.
- Deliver skilled nursing services per prescribed orders.
- Monitor and interpret patient signs, symptoms, and changes.
- Collect and analyze healthcare data.
- Document patient information.
Qualifications
- Current LPN/LVN license.
- Valid TB or Chest X-Ray.
- BLS certification.
- Nursing experience (preferably with tracheostomy and ventilator care).
Skills
- Strong assessment, critical thinking, and documentation abilities.
- Ability to work independently and collaboratively.
- Proficiency in patient care procedures.
Key Duties
- Provide temporary skilled nursing care in patients' homes.
- Support recruitment and training of full-time caregivers.
- Ensure quality care during the transition period.
