LPN Medication Care Manager – Senior Living

Responsibilities

• Administer medications and treatments safely per physician orders
• Follow the Six Rights of Medication Administration
• Accurately document in MAR/TAR
• Monitor symptoms and report adverse reactions
• Conduct Service & Health Updates
• Transcribe orders
• Observe and report physical, emotional, or cognitive changes
• Maintain medication carts
• Ensure compliance standards

Qualifications

• Active LPN license
• CPR and First Aid certification
• Strong assessment and documentation skills
• Senior care or medication administration experience preferred
