Responsibilities
- Provides considerate, respectful care focused on residents' individual needs
- Affirms residents' rights to make decisions regarding medical care, including discontinuing treatment as permitted by law
- Obtains and interprets information based on residents' age-specific needs
- Demonstrates understanding of care needs across the life span and completes life-span education as required
- Oversees resident/patient care on unit, conducting and coordinating the development and completion of Resident Assessments (RAI), including the Minimum Data Set (MDS) care plan, Medicare certification, and care plan conferences in accordance with federal, state, and facility standards
- Conducts and coordinates scheduled and unscheduled MDS assessments, care plans, and care plan conferences
- Ensures completion of MDS per resident assessment instrument (RAI) guidelines, including dating and signing each MDS (RN must sign)
- Monitors MDS and care planning documentation for all residents; ensures correct diagnosis documentation supports MDS coding
- Ensures MDS information is transmitted per guidelines
- Participates in Medicare utilization review (UR) meetings and triple check meetings to review residents’ progress and determine skilled services qualifications
- Coordinates Assessment Reference Date (ARD) with nursing and therapy to determine PDPM/RUG categories for each resident
- Obtains prior authorizations for managed care residents
- Participates in Interdisciplinary Team meetings, Licensed Nursing Meetings, and other necessary meetings
- Ensures SNF care plans are established and reviewed in accordance with regulatory guidelines and reflect residents' current conditions
- Reviews care plans with residents/designated parties quarterly or as indicated
- Coordinates the MDS schedule with the Interdisciplinary Team to ensure compliance
- Participates in daily stand-up meetings and completes resident follow-ups as requested
- Assures quality nursing care and follow-up with CNAs and nursing staff
- Informs Director of Nursing (DON) of persistent non-compliant documentation issues
- Coordinates with finance and therapy management on CMS audits, managed care audits, and demand bills
- Participates in quality assurance and performance improvement (QAPI) meetings, including monitoring, audits, and investigations
- Participates in facility surveys, inspections, and audits to ensure regulatory compliance
- Generates CASPER reports monthly and provides analysis for areas above acceptable standards
- Tracks and completes level of care determinations (LOCD) and user-defined assessments (UDA) for compliance
- Demonstrates sound nursing judgment in critical and resident care management situations
Requirements
- Graduate of an approved LPN program
- Previous experience completing the MDS
- Current NJ LPN license
- BLS certification
- Satisfactory completion of the pharmacology test
- Strong professional, organizational, and interpersonal skills

Pay
Compensation Range: $33.00 - $52.80 per hour. The range reflects base salary and does not include extra shift rates or incentives tied to quality or productivity. Compensation may vary based on education, experience, skills, licenses, certification, and training.
