Job Summary
The Licensed Practical Nurse (LPN) in the Rehabilitation Unit provides nursing care within the scope of licensure to patients requiring rehabilitative support under the supervision of a Registered Nurse (RN). The LPN delivers quality care through collaboration with the healthcare team, supports recovery and mobility efforts, and ensures compliance with clinical standards and hospital policies.
Essential Functions
- Provide direct patient care, administer medications, and perform treatments under RN supervision
- Monitor and document patient symptoms, responses, and progress
- Absorb and perform rehabilitation therapies and activities per patient care plan
- Support pain management and educate patients/families on coping strategies
- Report patient condition changes and consult with RN regarding care needs
- Perform procedures such as specimen collection and vital signs
- Interact respectfully and professionally with patients, families, and staff
- Identify and address psychosocial, cultural, and spiritual needs of patients
- Ensure patient safety and comply with infection control practices
- Participate in care planning, patient education, and discharge preparation

Qualifications
- Education: Graduate of an accredited practical/vocational nursing program.
- Licenses/Certifications: Current state licensure as a Licensed Practical/Vocational Nurse, Basic Life Support (BLS) certification (required), De-escalation/Crisis Mgmt. training (required within time frame specified in facility policy).
