LPN/LVN - On Call

About the role

Vi at Grayhawk is located at 7501 E. Thompson Peak Pkwy, Scottsdale AZ 85255. As a Licensed Practical Nurse (LPN) you will deliver care and services to residents by identifying resident needs, implementing prescribed interventions, and evaluating outcomes and conditions. This position also provides personal care for residents as needed and collaborates with the care team to resolve resident care problems, enhance care delivery, and participate in quality assessment and performance improvement activities.

Responsibilities

• Deliver care and services to residents through identification of resident needs and implementation of prescribed interventions.
• Evaluate outcomes and conditions of residents.
• Perform personal care for residents as needed.
• Communicate and collaborate with other members of the care team.
• Resolve resident care problems and enhance care delivery.
• Participate in quality assessment and performance improvement activities.

Requirements

• Graduation from a School of Nursing for Licensed Practical/Vocational Nurses.
• Current CPR and Automated External Defibrillator (AED) certifications.
• Current First Aid certification or willingness to become certified.
• Minimum 6 months of experience working as a Licensed Practical/Vocational Nurse (preferred).
• Prior experience working with geriatric clients (preferred).
• Food handlers certification, where required.
• Basic computer skills and/or willingness to learn.

Pay

USD $27.57 - USD $34.47 per hour.
