LPN/LVN Nurse

Responsibilities

• Utilize the nursing process to plan, implement, and evaluate patient care
• Administer medications and treatments
• Monitor and document patient progress
• Intervene and report changes in patient condition
• Document care coordination
• Ensure patient safety and equipment effectiveness

Training & Support

• Participate in a specialized training program with mentorship from experienced nurses
• Gain hands-on, one-on-one patient care experience in a home setting
• Learn vent, trach, and g-tube management
• Develop critical thinking skills with 24/7 clinical support

Qualifications

• Graduate of an accredited nursing program
• Active LPN/LVN license
• Current CPR certification
• PPD or chest X-ray if applicable
• Committed to working directly with patients at least 24 hours per week
