Responsibilities

  • Provide patient care and basic nursing tasks within LTC setting per protocols.
  • Monitor patient status; document vitals, changes, and care plans accurately.
  • Administer medications and treatments as prescribed; track adherence and side effects.
  • Ensure compliance with safety, infection control, and regulatory standards.
  • Collaborate with interdisciplinary team to update care plans and discharge planning.
  • Identify and report clinical changes early; escalate to supervising RN as needed.
  • Demonstrate accountability for personal performance; meet individual targets and quality metrics.
  • Support professional development through ongoing learning and skill enhancement.
  • Maintain organized patient records, optimize workflow, and improve care continuity.
  • Engage with residents and families to provide compassionate communication and education.
  • Participate in daily huddles and pipeline reviews to forecast care needs and staffing.
  • Auxiliary in monitoring pipelines for potential admissions and transfers within territory.

Requirements

  • Current LPN license with active-practicing permission.
  • Proven LTC or long-term care nursing experience preferred.
  • Ability to document vitals, care plans, and medications accurately.
  • Competence in medication administration and monitoring for side effects.
  • Strong clinical assessment skills and early escalation to RN when needed.
  • Knowledge of safety, infection control, and regulatory standards.
  • Excellent communication with residents, families, and interdisciplinary team.
  • Ability to work in a performance-driven, metrics-focused environment.
  • Proactive participation in daily huddles, pipeline reviews, and staffing planning.
  • On-site availability with reliable time management to meet targets.

Qualifications

Current LPN license with active-practicing permission.
