LPN - LTC

Job Summary The LPN LTC provides nursing care to residents in a skilled nursing facility (SNF) or long-term care setting. The nurse monitors residents' health, administers medications, assists with daily care, documents patient information, and coordinates with registered nurses and other healthcare professionals. Key Responsibilities Provide nursing care to residents in a long-term care facility.Administer medications and treatments as prescribed.Monitor vital signs and changes in residents' health.Document nursing care using the MatrixCare charting system.Assist residents with daily care needs and promote safety.Communicate with physicians, RNs, and other healthcare staff.Follow infection-control procedures and facility policies. Requirements Active New York State LPN license mandatory.BLS certification mandatory.Minimum 2 years of LPN experience in a skilled nursing facility or long-term care setting.Previous travel nursing experience required, preferably with contract renewals.Experience with MatrixCare charting preferred.
