About the role

Complete Care at Care Age is seeking a dedicated Licensed Practical Nurse (LPN) to serve as a clinical bedside leader, ensuring high-quality resident care and precise execution of physician orders.

Responsibilities

• Medication Management: Safely prepare and administer medications and treatments following established safety protocols.
• Order Transcription: Accurately transcribe physician orders and coordinate seamlessly with pharmacy providers.
• Clinical Monitoring: Track resident health status, perform routine assessments, and promptly report changes to the RN or DON.
• Documentation Excellence: Complete accurate, timely daily charting in the electronic health record (EHR).
• Interdisciplinary Teamwork: Collaborate with CNAs, RNs, and therapists to maintain excellent continuity of care and compliant unit operations.

Qualifications

• License: Must possess a current, active, and unrestricted Licensed Practical Nurse (LPN) license in the State of Wisconsin.
• Skills & Abilities: Strong organizational skills, excellent attention to detail, time management, and a compassionate approach to care.
• Experience: Previous long-term care or skilled nursing experience is preferred, but all dedicated LPN applicants are encouraged to apply.

Pay & Schedule

• Sign-On Bonus: $5,000 for Full-Time positions.
